چکیده

Among the various ways of managing poisoning, haemodialysis may help in enhancing excretion of the toxic substance. We report a case, a Russian male, 35 years old, who was rushed to the Gleneagles Hospital Medan from the airport after being evacuated from Banda Aceh together with another older Russian who died as they arrived at the admission and Emergency Department. From the result of intensive allo anamnesis and the high anion gap metabolic acidosis, in the absence of disturbed renal and liver function, we presumed this patient was suffering from methanol intoxication. The time of exposure was approximately 70 hours before. The exact length of dialysis time to excrete the noxious substance from the blood without plasma methanol determination was difficult. Moreover the time elapsed from exposure to treatment had been approximately 70 hours, which means the optic nerve had been so long exposed to formic acid, the toxic metabolite of methanol, that the damage should have been very severe. Ethanol is also known to be an antidote of methanol, which can be given orally by nasogastric tube, or i.v. It should be given early, and plasma ethanol level should be closely monitored to make it effective and safe. This was also unavailable. Another antidote is fomepizole which is also as yet unavailable in Medan. Folic acid, thiamin, and i.v. folinic acid are also recommended by the literature, as well as oral steroid.

Recommendations for the role of extracorporeal treatments in the management of acute methanol poisoning: a systematic review and consensus statement.

OBJECTIVE Methanol poisoning can induce death and disability. Treatment includes the administration of antidotes (ethanol or fomepizole and folic/folinic acid) and consideration of extracorporeal treatment for correction of acidemia and/or enhanced elimination. The Extracorporeal Treatments in Poisoning workgroup aimed to develop evidence-based consensus recommendations for extracorporeal treat...

Effect of Extracorporeal Shockwave Therapy Versus Stretching in the Treatment of Athletes With Chronic Plantar Fasciitis

Purpose: Extracorporeal shockwave therapy (ESWT) is a relatively new method in the treatment of musculoskeletal disorders such as plantar fasciitis. This study aimed to compare two methods of treatment with extracorporeal shockwave and stretching in athletes with chronic plantar fasciitis. Methods: The study was a randomized clinical trial. A total of 30 male athletes with chronic plantar fasc...

Antibiotic prophylaxis in the prevention of urinary tract infection in patients with sterile urine before extracorporeal shock wave lithotripsy

Background: One of the lithotripsy complications is urinary tract infection (UTI) and sepsis after extracorporeal shock wave lithotripsy (ESWL). The aim was to study the prophylactic effect of antibiotics on UTI after ESWL. Methods: This randomized double-blind clinical trial was carried out on 600 patients admitted to Babol Clinic hospital in 2014-2015. Patients were randomly divided into tre...

The Effect of Extracorporeal Shock Wave Therapy on Clinical and Functional Outcomes of Individuals with Medial Tibial Stress Syndrome: A Systematic Review

Background and Aim: Medial Tibial Stress Syndrome (MTSS) is one of the most common sports-related overuse injuries among runners and military personnel. Extracorporeal shock wave therapy (ESWT) is an emerging treatment modality that used for individuals with MTSS. However, there was no found a systematic review that specifically assess the clinical and functional efficacy of ESWT in individuals...

Emerging technologies in extracorporeal treatment: focus on adsorption.

As an extracorporeal technique for blood purification, haemoadsorption was introduced in the early 1960s along with other physico-chemical methods. The problem of poor biocompatibility of uncoated adsorbents was resolved by coating adsorbent granules with haemocompatible membranes. Use of coated adsorbents instead of uncoated ones reduces the efficiency of haemoperfusion. As a result, for many ...

Tracheal Chondrosarcoma Excised With Extracorporeal Circulation

Introduction: Tracheal chondrosarcoma (TCS) is a rare malignancy, with only 19 cases described in the literature to date. Case presentation: Herein, we presented the third-largest TCS with such an airway compromise that neither orotracheal intubation nor jet ventilation or even tracheostomy was possible. So, extracorporeal circulation was needed to excise the tumor in a one-stage procedure. Th...
